Voor deelname aan de Belgische Transplantoux Games moet de algemene gezondheid en conditie van je kind stabiel zijn, zoals beoordeeld door jou en bevestigd door de opvolgende (transplantatie- of dialyse) arts van je kind. Je zal gevraagd worden een verklaring te ondertekenen waarin je bevestigt dat je kind fit is om deel te nemen aan de gekozen sport(en) en dat je beschikt over een gepaste medische verzekering die zijn/haar deelname aan de Belgische Transplantoux Games dekt.
Before competing in the Belgian Transplantoux Games your child's general health and fitness must be stable as judged by yourself and confirmed by the child's transplant or dialysis follow-up doctor. You will be asked to sign a waiver to confirm that your child is fit to compete in the chosen sport(s) and have appropriate medical insurance to cover the participation in the Belgian Transplantoux Games.
Ik ga ermee akkoord dat medische gegevens worden overgedragen naar een online systeem, zodat het medisch/fysiotherapieteam van de Belgische Transplantoux Games 2025 er toegang toe heeft en deze kan gebruiken, uitsluitend met het doel om, indien nodig, tijdens de duur van de Games medische zorg te verlenen.
I agree that medical data captured will be transferred to an online system for access and use by the Belgian Transplantoux Games 2025 medical / physio team for the sole purpose of providing treatment, if required, for the duration of the Games.
